The Community Liaison serves as the vital link between Giving Home Health Care (GHHC) and the patients we support. In this role, you will connect directly with patients, families, and caregivers to ensure their needs are met and concerns are resolved promptly. You’ll maintain accurate documentation, communicate clearly, and collaborate closely with care teams to address issues and deliver exceptional patient experience. #IND
