Renal dose checks should happen before the refill feels routine
For medications with renal dosing changes, a stable refill history does not remove the need to check the newest kidney function, recent acute illness, and interacting medications. The safest intervention often happens before the patient reports a problem.
Ask about access before assuming nonadherence
When a patient misses therapy, separate intentional nonadherence from access barriers. Cost, transportation, stock issues, prior authorization delays, and health literacy can all look like refusal unless the team asks directly.
