What you'll do
- Serve as the primary point of contact for patients throughout the Initial Home Health Care admissions and onboarding process, ensuring consistent communication, support, and advocacy.
- Manage an independent caseload of patients through the admissions, authorization, and onboarding process while maintaining timely workflow progression and follow-up.
- Coordinate physician appointments, nursing assessments, home visits, caregiver onboarding, and required documentation to support timely initiation of home health care services.
- Conduct home visits to establish patient relationships, provide education regarding the home health program, assess patient needs, gather documentation, and support onboarding activities.
- Attend physician appointments as a patient advocate, facilitating communication between providers, patients, and internal departments while obtaining required signatures, documentation, and physician orders.
- Collaborate with patients, families, physician offices, Resource Nurses, Authorization Specialists, Writing, Staffing, and operational departments to support continuity of care and timely workflow progression.
