What you'll do
- Support Sana members in navigating the healthcare system and accessing appropriate, high-quality, cost-effective care.
- Help members find providers, understand referral options, and coordinate access to healthcare services.
- Communicate with members, healthcare providers, and external partners to gather information, resolve routine issues, and facilitate care.
- Collaborate with Care Navigators, Sana Care providers, Medical Assistants, and Customer Support to ensure a seamless member experience.
- Maintain accurate, timely documentation of member interactions and care navigation activities.
- Follow established workflows, manage tasks through completion, and escalate complex clinical, benefits, or operational questions to appropriate team members.
